If you have made a purchase of an FDA prescription-only product (medical devices and masks both require a US prescription as-per the FDA.

To make a purchase of an FDA prescription-only product, complete our online prescription form to securely submit your prescription to Oxy-Pam. Medical devices and masks both require a US prescription as per FDA regulations, and we cannot accept prescriptions from non-US doctors.

To submit by the secure online form scan or take a photo of your prescription and submit the image and your order ID.

  • You can find your Order Number on the email you got when you order your product.
  • Please attach a scanned image of your prescription showing the prescribed pressure settings.
